Detailed Information
The thyroid gland produces hormones that influence metabolism, energy levels, body temperature, heart rate, growth, digestion, and many other physiological processes. T4 is the principal hormone released by the thyroid, while T3 is the more biologically active hormone. TSH is produced by the pituitary gland and regulates thyroid hormone production through a feedback mechanism.
Total T3 and total T4 measurements include both protein-bound and unbound hormones circulating in the blood. FT3 and FT4 measure the unbound portions that are available to enter tissues and produce biological effects. Measuring both total and free hormones may provide a broader assessment when binding-protein changes or complex thyroid findings are suspected.
TSH is a sensitive marker of the interaction between the pituitary gland and the thyroid. An increased or decreased TSH result may indicate altered thyroid activity, but it must be interpreted together with T3, T4, FT3, FT4, symptoms, medication use, and other clinical information.
Thyroid peroxidase is an enzyme involved in the production of thyroid hormones. Anti-TPO antibodies develop when the immune system mistakenly targets this enzyme. Elevated Anti-TPO levels are frequently associated with Hashimoto's thyroiditis and may also occur in Graves' disease or other autoimmune thyroid conditions.
Thyroglobulin is a protein produced by thyroid cells and used in thyroid hormone formation. Anti-TG antibodies are produced when the immune system reacts against thyroglobulin. Testing Anti-TG alongside Anti-TPO provides additional information during the investigation of suspected thyroid autoimmunity.
Hypothyroidism may cause fatigue, weight gain, constipation, dry skin, hair thinning, cold sensitivity, slow heart rate, low mood, and menstrual irregularities. Hyperthyroidism may cause weight loss, rapid heartbeat, tremors, anxiety, sweating, heat intolerance, disturbed sleep, and increased bowel movements.
Detectable thyroid antibodies may indicate an autoimmune response against thyroid tissue, but they do not independently show how well the gland is currently functioning. Some individuals may have thyroid antibodies while their hormone levels remain within the laboratory reference intervals.
Results should always be interpreted alongside clinical history, symptoms, physical examination, pregnancy status, medication and supplement use, imaging findings, and physician evaluation. Repeat testing or additional investigations may be recommended depending on the findings.
