About Thyroid Testing
The thyroid is a small gland in the front of the neck that makes hormones controlling metabolism. When it is underactive (hypothyroidism) things slow down: tiredness, weight gain, feeling cold, constipation, dry skin, low mood and heavy or irregular periods. When it is overactive (hyperthyroidism) things speed up: weight loss, palpitations, anxiety, tremor and heat intolerance.
In India, the most common cause of hypothyroidism is autoimmune thyroiditis (Hashimoto's disease), in which the immune system gradually damages the gland. Thyroid antibody tests can show this even before hormone levels change.
Thyroid balance matters most in pregnancy, when the baby depends on the mother's hormones for brain development. That is why TSH is part of routine antenatal tests.

What the key tests tell you
- TSH
- The single best screening test. High TSH usually means an underactive thyroid; low TSH an overactive one.
- T3 and T4 / free T3 and free T4
- The thyroid hormones themselves. Free hormone levels help when TSH is abnormal or in pregnancy.
- Anti-TPO and anti-thyroglobulin antibodies
- Show autoimmune thyroid disease, and predict who is likely to develop hypothyroidism over time.
- TSH receptor antibody (TRAb)
- Confirms Graves' disease, a common cause of an overactive thyroid.
- Thyroid ultrasound
- Examines nodules and swelling, and helps decide whether a nodule needs further tests.
Before your test
- No fasting is needed for thyroid tests, but an early-morning sample gives the most consistent results.
- If you take thyroid tablets, give the sample before your daily dose unless your doctor says otherwise.
- Stop biotin (often in hair and nail supplements) two days before, as it can distort thyroid results.
Understanding your results
A slightly raised TSH with normal T4, called subclinical hypothyroidism, often just needs a repeat test after six to eight weeks. Treatment depends on the level, symptoms, antibodies and pregnancy plans.
Once on treatment, TSH is usually checked six to eight weeks after any dose change, then every six to twelve months when stable.

Do I need to fast for a thyroid test?
No. Fasting is not needed, though a morning sample before your thyroid tablet gives the most reliable result.
What is a normal TSH level?
Most labs use a reference range of about 0.4 to 4.5 µIU/mL for adults, with different targets in pregnancy. Your report shows the range used, and your doctor interprets it.
Can thyroid problems cause weight gain?
An underactive thyroid can cause modest weight gain and make weight harder to lose. Treating it helps, but most weight change has other causes too.
