About Breast Cancer Screening
A mammogram is a low-dose X-ray of the breast that can show cancers too small to feel, as well as tiny calcium deposits that can be an early sign. Breast ultrasound is added for women with dense breast tissue, for younger women, and to examine a lump.
Many women in India still come for testing only after they find a lump. Regular screening changes that: cancers found on screening are usually smaller, less likely to have spread, and need less extensive treatment.
Most breast lumps are not cancer. Cysts and fibroadenomas are common, especially in younger women. Even so, any new lump, nipple discharge or skin change should be checked.
Who should get tested
- Women from about age 40, every one to two years, as advised by your doctor
- Earlier screening for women with a mother, sister or daughter diagnosed young, or known BRCA changes
- A new lump or thickening in the breast or armpit
- Nipple discharge, especially blood-stained, or a nipple turning inward
- Dimpling, redness or orange-peel texture of the breast skin
- Breast pain in one spot that does not go away with the cycle

What the key tests tell you
- Digital mammography
- Low-dose X-ray images of both breasts, the standard screening test from about 40.
- Breast ultrasound
- Examines lumps, separates cysts from solid lumps and complements mammography in dense breasts.
- BI-RADS reporting
- A standard scoring system on the report that tells your doctor how likely a finding is to need further tests.
- Biopsy referral
- When a finding needs it, a sample of tissue is the only way to confirm or rule out cancer.
- CA 15-3
- A tumour marker used to monitor known breast cancer. It is not a screening test.
Before your test
- Book the mammogram for the week after your period, when breasts are least tender.
- Do not apply deodorant, powder or cream on the chest or underarms on the day of the test.
- Wear a two-piece outfit so you only need to remove the top.
- Bring previous mammograms and ultrasound reports, as comparison is very useful.
Understanding your results
Reports use the BI-RADS system. Category 1 or 2 means normal or clearly benign; category 3 usually means a short-interval follow-up; categories 4 and 5 need a biopsy. Being called back for extra views is common and most recalls turn out normal.
A normal mammogram does not mean you can ignore a new lump. Always report changes you notice, even between screening tests.

At what age should I start mammography?
Generally from about 40, every one to two years. Women with a strong family history may need to start earlier with additional tests, on a doctor's advice.
Is a mammogram painful?
The breast is pressed firmly for a few seconds to get clear images, which can be uncomfortable but is brief. Scheduling after your period reduces discomfort.
Is mammography radiation safe?
The radiation dose is very low and the benefit of finding cancer early far outweighs the small risk for women in the screening age group.
Do I need a mammogram or an ultrasound?
Women over 40 usually start with mammography, with ultrasound added for dense breasts or a lump. Younger women with a lump usually start with ultrasound.
