About Bone Health Testing
Bone is living tissue that is constantly broken down and rebuilt. After about 30 the balance gradually tips towards loss, and in women it speeds up for several years after menopause. Osteoporosis is the result when bones become thin and fragile.
Vitamin D deficiency, very common in India, reduces calcium absorption and weakens bones. Low protein intake, inactivity, smoking, long-term steroid use, thyroid problems and some other conditions add to the risk.
Joint pain is a different but related concern. Blood tests help separate wear-and-tear arthritis from inflammatory types such as rheumatoid arthritis and from gout, which is caused by uric acid crystals.
Who should get tested
- Women over 65 and men over 70, or younger with risk factors
- Women after menopause, especially early menopause
- A fracture after a minor fall, or loss of height
- Long-term steroid tablets, or thyroid, kidney or digestive disorders
- Low body weight, or a parent who had a hip fracture

What the key tests tell you
- BMD (DEXA) scan
- Measures bone density at the hip and spine. The T-score shows whether bones are normal, thinning (osteopenia) or osteoporotic.
- Vitamin D (25-OH)
- Low levels are common and weaken bones; correcting them is the first step in bone care.
- Calcium, phosphorus and alkaline phosphatase
- Basic minerals and a bone-turnover marker; abnormal patterns point to vitamin D or parathyroid problems.
- Parathyroid hormone (PTH)
- Checked when calcium is abnormal; the parathyroid glands control calcium balance.
- Uric acid
- High levels can cause gout, which typically produces sudden, very painful joint swelling.
- RA factor, anti-CCP, ESR and CRP
- Help identify inflammatory arthritis when joints are swollen and stiff.
Before your test
- For a BMD scan, avoid calcium supplements on the day of the test and wear clothes without metal zips or buttons.
- Tell us if you have had a barium study or a nuclear scan in the past two weeks, as it can affect the BMD result.
- No fasting is needed for vitamin D; calcium and phosphorus are best done fasting in the morning.
Understanding your results
A T-score of −1 or above is normal, between −1 and −2.5 is osteopenia, and −2.5 or below is osteoporosis. Your doctor combines this with age and other risk factors to decide on treatment.
Vitamin D and calcium alone do not treat established osteoporosis, but they are essential alongside any medicine. A repeat BMD scan, usually after one to two years, shows the response.
