About Toxicology and Drug Level Testing
Some medicines work only within a narrow range: too little and they fail, too much and they cause side effects. Anti-seizure drugs such as phenytoin, valproate and carbamazepine, lithium, digoxin and some immunosuppressants are monitored with blood levels so the dose can be adjusted safely.
Drugs-of-abuse screening is requested for clinical, rehabilitation or employment reasons. Urine panels detect commonly misused substances, and positive screens can be confirmed with more specific methods when required.
Heavy metals such as lead, mercury and arsenic can build up through occupational exposure, contaminated water, some traditional remedies and old paint. Children are especially sensitive to lead, which can affect learning and behaviour.

- Therapeutic drug levels

Before your test
- For drug level tests, the timing matters. A 'trough' level is usually taken just before the next dose. Follow your doctor's instructions and note the time of your last dose.
- Bring a list of all medicines, supplements and traditional remedies you take.
- For drug screening, a supervised urine sample may be required. Carry a photo ID if the test is for employment.
Understanding your results
Drug levels are interpreted against a therapeutic range and your symptoms; your doctor may change the dose based on the result. Do not change medicines on your own.
A positive screening result for drugs of abuse may need confirmation, because some medicines and foods can cross-react. Results are handled confidentially.
