About Anemia Testing
Anemia means the blood has less haemoglobin than it should, so less oxygen reaches the body's tissues. Mild anemia may cause no symptoms; as it worsens it brings tiredness, breathlessness on climbing stairs, pale skin, palpitations and headaches.
Iron deficiency is the commonest cause, from low dietary intake, heavy periods, pregnancy, or slow blood loss from the stomach or bowel. Vitamin B12 and folate deficiencies, chronic kidney disease, long-standing illness and inherited conditions such as thalassaemia trait are other important causes.
Taking iron without knowing the cause can delay finding the real problem. For example, thalassaemia trait looks like iron deficiency on a basic blood count but does not respond to iron.

What the key tests tell you
- Complete blood count (CBC)
- Measures haemoglobin and red cell indices such as MCV, which show whether red cells are small, normal or large and point to the likely cause.
- Peripheral smear
- A pathologist examines red cells under the microscope for shape and size changes typical of particular anemias.
- Iron studies and ferritin
- Ferritin is the best single measure of iron stores. Serum iron, TIBC and transferrin saturation complete the picture.
- Vitamin B12 and folate
- Deficiency causes large red cells and can also affect nerves.
- Haemoglobin electrophoresis (HPLC)
- Screens for thalassaemia trait and other inherited haemoglobin variants.
- Reticulocyte count
- Shows whether the bone marrow is producing new red cells, which helps separate production problems from blood loss.
Before your test
- No fasting is needed for a CBC; iron studies are best done in the morning, ideally fasting.
- If you have started iron or vitamin tablets, mention it, because they can change results.
- Bring previous blood counts so changes over time can be seen.
Understanding your results
Haemoglobin below about 13 g/dL in men and 12 g/dL in women is generally considered anemia, with lower thresholds in pregnancy. The type of anemia shown by the indices and iron tests guides treatment.

Is a CBC enough to check for anemia?
A CBC confirms anemia and gives clues to its type. Iron studies, ferritin, B12, folate or HPLC are added to find the exact cause.
What is the difference between iron deficiency and thalassaemia trait?
Both cause small red cells, but thalassaemia trait is inherited and does not need iron. HPLC testing tells them apart, which matters for treatment and for family planning.
How long does iron treatment take to work?
Haemoglobin usually starts rising within a few weeks, but iron stores take about three months to refill. Your doctor will repeat tests to check progress.
