About Liver Function Testing
The liver processes nutrients and medicines, makes proteins, clears toxins and produces bile. It has a large reserve, so damage often goes unnoticed until it is advanced. Liver enzyme tests are the early warning.
Non-alcoholic fatty liver disease is closely linked to weight gain, diabetes and high triglycerides, and is now the leading cause of abnormal liver tests. Alcohol, viral hepatitis B and C, certain medicines and some herbal products are other common causes.
Most early liver problems improve when the cause is addressed. Losing weight can reduce liver fat substantially, and hepatitis B and C now have very effective treatments.
Who should get tested
- Overweight, diabetes or high triglycerides (fatty liver risk)
- Regular alcohol use
- Yellow eyes or skin, dark urine, itching or pale stools
- Pain or heaviness in the upper right abdomen, or loss of appetite
- Long-term medicines such as anti-TB drugs, anti-seizure drugs or statins
- Family member with hepatitis B, or past blood transfusion

What the key tests tell you
- SGPT (ALT) and SGOT (AST)
- Enzymes released when liver cells are injured. ALT is the more liver-specific of the two.
- Bilirubin (total and direct)
- Rises in jaundice; the pattern helps separate liver causes from blood or bile-duct causes.
- Alkaline phosphatase and GGT
- Point towards bile-duct problems; GGT also rises with alcohol and some medicines.
- Albumin and total protein
- Show how well the liver makes proteins, which falls in long-standing liver disease.
- HBsAg and anti-HCV
- Screen for hepatitis B and C, which can damage the liver silently for years.
- Ultrasound abdomen
- Detects fatty liver, its grade, and other changes in the liver and gall bladder.
Before your test
- Fasting for 8–12 hours is often advised when LFT is done with lipids or sugar; follow the instructions given at booking.
- Avoid alcohol for at least 24 hours before the test.
- For an abdominal ultrasound, fast for about 6 hours so the gall bladder can be seen clearly.
- List all medicines and herbal supplements you take.
Understanding your results
Mildly raised liver enzymes are common and often due to fatty liver. Your doctor will look at the pattern, repeat the tests and look for the cause rather than treat the number alone.
If fatty liver is found, a combined score from blood tests (such as FIB-4) and, where needed, further assessment help judge whether there is any scarring.

What does an LFT test include?
Typically SGPT (ALT), SGOT (AST), alkaline phosphatase, GGT, bilirubin, total protein, albumin and globulin.
Is fasting required for a liver function test?
It is often advised, especially when done with lipid or sugar tests. Follow the instructions given when you book.
Can fatty liver be reversed?
In most people, yes. Gradual weight loss of 7–10%, regular exercise and avoiding alcohol can significantly reduce liver fat.
How is hepatitis B tested?
With an HBsAg blood test. If positive, further tests show whether the infection is active and whether treatment is needed.
