The thyroid gland is a butterfly-shaped gland located in the neck. It produces hormones that regulate metabolism, energy production, body temperature, heart rate, digestion, growth, and many other essential body functions.

Thyroxine, also known as T4, is the main hormone released by the thyroid gland. Most T4 circulating in the blood is attached to proteins. FT4 measures the small unbound portion that is available to enter body tissues and produce biological effects.

TSH is produced by the pituitary gland and regulates thyroid hormone production through a feedback mechanism. When thyroid hormone levels decrease, the pituitary gland generally releases more TSH to stimulate the thyroid. When thyroid hormone levels increase, TSH production is usually reduced.

An elevated TSH result accompanied by a reduced FT4 level may support the evaluation of an underactive thyroid. A reduced TSH result accompanied by an elevated FT4 level may support the evaluation of an overactive thyroid. Other result patterns may require additional investigation and clinical correlation.

Hypothyroidism may cause fatigue, weight gain, constipation, dry skin, hair thinning, cold sensitivity, slow heart rate, low mood, and menstrual changes. Hyperthyroidism may cause weight loss, rapid heartbeat, tremors, anxiety, sweating, heat intolerance, sleep difficulties, and increased bowel movements.

FT4 and TSH levels may be influenced by thyroid medication, other medicines, pregnancy, acute illness, pituitary disorders, and laboratory assay interference. Additional tests such as FT3, thyroid antibodies, or thyroid imaging may be recommended depending on the findings.
