About this test
The LIPID PROFILE is a blood test that measures different types of fats, known as lipids, circulating in the bloodstream. A standard profile commonly includes total cholesterol, triglycerides, high-density lipoprotein cholesterol and low-density lipoprotein cholesterol. Depending on the laboratory method, it may also report very-low-density lipoprotein cholesterol, non-HDL cholesterol and calculated cholesterol ratios.
Cholesterol is necessary for producing hormones, vitamin D and healthy cell membranes. However, an unhealthy lipid pattern can contribute to the formation of fatty deposits inside arteries. Over time, this process may narrow or block blood vessels and increase the risk of coronary artery disease, heart attack, stroke and peripheral arterial disease. High cholesterol frequently causes no noticeable symptoms, which makes laboratory screening important.
LDL is often described as bad cholesterol because elevated levels can promote cholesterol deposition in artery walls. HDL is commonly called good cholesterol because it helps transport cholesterol away from tissues for processing by the liver. Triglycerides are another form of fat used for energy, but persistently high levels may add to cardiovascular risk. Extremely elevated triglycerides can also increase the risk of acute pancreatitis.

Preparation Before Test
Fasting requirements depend on the reason for testing and the instructions of the treating doctor. Many routine Lipid Profiles can be performed without fasting. However, fasting for approximately 9 to 12 hours may be requested when triglycerides require careful assessment, when previous results were significantly abnormal or when the clinician specifically needs a fasting sample. During fasting, plain water is generally allowed.
Avoid an unusually heavy or fatty meal and excessive alcohol before testing because these may temporarily affect triglyceride values. Continue regular medicines unless the doctor provides different instructions. Inform the laboratory about current medicines, supplements, pregnancy, recent illness and any previous lipid results. Intense exercise shortly before sample collection may also influence certain measurements.

Total cholesterol represents the overall amount of cholesterol carried in the blood. LDL cholesterol is a major treatment target because higher concentrations are associated with atherosclerotic cardiovascular disease. HDL cholesterol is generally considered protective at appropriate levels, although an HDL result should not be interpreted independently. Triglycerides can rise due to recent food intake, alcohol consumption, poorly controlled diabetes, excess weight, hypothyroidism, kidney or liver disease, genetic conditions and certain medicines.
Non-HDL cholesterol is calculated by subtracting HDL from total cholesterol and represents cholesterol carried by potentially atherogenic particles. Some reports also include ratios such as total cholesterol to HDL cholesterol. These calculations may provide additional context, but treatment decisions usually consider the complete lipid pattern and the patient's overall cardiovascular risk.
Reference intervals and treatment targets are not identical for every person. Someone with established cardiovascular disease, diabetes, chronic kidney disease or very high calculated risk may be given a lower LDL target than a person at low risk. A single abnormal result does not always establish a persistent disorder. The doctor may recommend repeat testing, lifestyle changes, further evaluation for secondary causes or medication based on the degree of abnormality and the patient's risk profile.
